At the top, It says verification system for a name and social security number. I'm assuming they should have a registration system for foreigners. First blank is used for your name to fill out and others for your social security number. It says If you are attempting an identy theft.. you will be fined less than USD$12,000 or in behind the bars for about 3years at the bottom.

It's not the social security number, it is the Korean Citizenship number. Not the same thing.
